Friend of mine uses a paperback-sized whiteboard to make up a somewhat simpler form of this for every encounter. Just initiative totals, really, but very useful.
After seeing this I thought that having little tablet sized whiteboards for each player would be a great idea. Cut way down on the blizzard of paper at the table.
