Wearing my employer hat, I'd say no. You can demonstrate the skills that you used there (truck and forklift driving) from your other employment. Based on only two weeks, they can't give you much of a reference either -- they just havn't gotten the opportunity to know you that well.
I don't think including them gives you any sort of a positive. And the length of employment could be a big negative in the eyes of a prospective employer. Even though you have a very good reason, your application might be discarded before you get an opportunity to explain.
