I think the biggest failure (on so many levels) was T4. Rather than having the newly-minted Third Imperium expanding out into the howling wilderness of the Long Night, once again we were handed a sector full of fully populated, technologically advanced systems.
The BS will the computer program they used to generate Core Sector was another problem, but T4 should have been about brave Scouts plunging into mysteries dark and vast, at least at first.
