1. I think there'd be some weight given to the circumstances of twins.. obviously fraternal (one born with hair, the other without, for example) or one boy and girl might ease up on the worries. The kids would probably still carry some stigma as twins.
2. Given the high infant mortality rates and the need for large peasant families, fourth sons might be a little common. Stretch it out so it's the fourth son of a fourth son of a fourth son of a fourth son, and you could see some weirding. This would be a great place to get a Sorcerer from. Such births can reawaken ancient bloodlines.
