Let's see, I used to be a professional scorekeeper*, so I should be able to handle this.
Sandoval is out 5 (unassisted? I forget how the ball got to Andrus).
Ishikawa is safe on a fielder's choice, and (as I recall) advances to second on the error (E-5) charged to Andrus.
Winn, who started at third base, scores, and Ishikawa gets an RBI because you're not allowed to assume a double play. Ishikawa is assumed to have reached on the FC and would have gotten credit for the RBI even without the error; he doesn't get a hit. Charge an earned run to the pitcher.
(I'm not certain, but I think that if Ishikawa didn't advance to second, there's not actually a chargeable error on the play on account of the can't-assume-a-DP rule.)
Getting all that into the little scorecards they have at the park would be difficult. I had a fairly sophisticated score sheet, and even then you got into complex situations where it was difficult to figure out where to put what. *When I was a sophomore in high school, I kept score and did PA announcements in the city of Bishop's rec league, for which the city paid me $4/game. Ergo, I'm a professional, albeit it was almost thirty years ago.
